The rc manpage says /lib/rc/init.d is mounted tmpfs where available. AFAICT, that's obsolete info, as neither mount nor /proc/mounts seem to know anything about it, and mount-binding the rootfs elsewhere (I have an entry for that to make backing up easier) shows the same files on the bind as on the original mount, so it doesn't appear to be tmpfs mounted after all.
FWIW my rootfs is reiserfs, so there's little reason not to do the initial mount r/w and I do just that, from the grub kernel command line, if that makes any difference.

Yes, it's because you mounted / rw at boot time. I've updated the manpage accordingly.
